After 25 years in practice, I am writing to inform you that after December 24th 2009 I will no longer be seeing patients at my ENT practice in Longview. I will be available to serve your medical needs until that date. After that date I will resume my practice in Tacoma at the Franciscan Medical Group, St. Josephs Hospital, Tacoma Ear & Balance Clinic – 253.426.4744

It is very important that you make arrangements as soon as possible to select a new ENT physician to make sure you receive uninterrupted medical care. Doctors Treyve and Davis of ENT of the Northwest are good ENT physicians and would be a good choice for your ENT needs.

Once you have selected a new physician, you will need to complete an authorization to release your medical records so that I can forward a copy of your records to your new physician. Unfortunately, your medical records cannot be released without a written authorization from you. You can pick up an authorization form at our office at 820 11th Ave. Longview, WA, fill it out and we will forward a copy of your record to your new ENT physician. Assuming I receive your completed authorization form prior to my last day of practice, there will not be a charge for copying your record. All authorization forms received after December 24th 2009 will be subject to a reasonable copying fee to cover the cost of duplication.
